Application Forms

The following are forms that those applying for positions at Services facilities may need as part of their application.

OF-612 and OF-612 Supplement are The Application for Federal Employment form and must be submitted for each position applied for, résumé's may be attached.

OF 306 Declaration for Federal MUST accompany the application package.

Anyone applying for a position who will be working directly with children must fill out Supplemental Child or Youth Appendix B and C.
